Many have been using Google forms for a long time, it is free, fast and reliable.  Surveymonkey is  a platform specializing in online surveys, I worked there for a while in the paid version, there are a lot of good things.

I make a survey for each new customer on the site, the one they want with a choice of answer options. I have been doing something similar on my resource for many years. One question, and two answers are interesting to visitors on my site or not. Moreover, I see statistics on when and what was clicked there.
